What is Chronic Care Management

In 2015, Medicare implemented a series of major reimbursement changes impacting beneficiaries with two or more chronic conditions expected to last 12+ months or until the death of the patient. Using Current Procedural Terminology (CPT) code 99490, Medicare will now reimburse at a rate of approximately $42 per month for each patient receiving 20 minutes or more of non-face-to-face care coordination services fulfilled by the provider or a subcontractor.
